You don't have to use a spacer, but the turbo will really close to the heat shield. Mine is about 1/4" or less of clearance which isn't much. A spacer would give you more clearance and you won't run the possibility of heat soaking the manifold. The only thing I had to change with the new turbo was the intake pipe, you'll have to enlarge it to fit onto the compressor.
